The process of determining need and our assistance includes application,
interview, and verification.
Items needed for the
process of application and interview include:

Photo Identification in the form of a picture ID

Social Security number in printed form (Social Security or ITIN card)
for each household member

Proof of current address (printed on a bill or a piece of mail)

Proof of the crisis such as utility cut off notice or eviction notice
-
Documentation of any income for the last 30 days

Documentation of expenses for the last 30 days (receipts, bank
statement)
Other verified information may be necessary in order for Sunnyside to
make an informed decision about the amount of assistance to provide.
